Agreement

I confirm that my health information is accurate and complete.

I understand that tattooing involves risks including infection, allergic reaction, scarring, and unpredictable healing outcomes. No specific results are guaranteed; outcomes depend on my skin type and how I heal.

I accept responsibility for these risks and release the artist and studio from liability for outcomes that depend on my biology or my adherence to aftercare.

I understand I will receive aftercare instructions at the end of my session and agree to follow them. The studio is not responsible for complications caused by failure to follow aftercare.

I understand that my personal and health information will be stored securely by the studio for a minimum of 10 years as required by Ontario regulations, and used for purposes of providing tattoo services, legal compliance, and portfolio documentation (including potential use of tattoo photographs on the studio's social media @archive.ottawa).
